CCC Awards Survey

We want to celebrate the team members that go above and beyond for our patients, around the workplace, and in our communities. Nominations will be reviewed by the leadership team and the winners will be announced at our quarterly all-staff meeting.

Care, Culture, and Community Awards

1. Care – This award is for the team member that goes above and beyond to increase patient outcomes. Please nominate one team member (Provider or Non-Provider) and include examples as to why you’re nominating this individual.

2. Culture – This award is for the team member that exudes enthusiasm and is passionate about our culture here at Viverant. Please nominate one team member (Provider or Non-Provider) and provide examples as to why you’re nominating this individual.

3. Community – This award is for the team member that represents Viverant through stewardship in their community. Please nominate one team member (Provider or Non-Provider) and provide examples as to why you’re nominating this individual.
